and participate in the events and activities listed below, which the North Jersey Home Schoolers Association Inc. has created, organized, scheduled and/or assisted with.
RELEASE
I, hereby freely and voluntarily release, both now and forever, the North Jersey Home Schoolers Association Inc., its directors, teachers, assistants, volunteers, and chaperones, from any and all liability, claims, demands, and or suits for personal injury and or property damage which arise out of the participation of the child in the event listed below, whether the personal injuries and or property damage results in whole or in part from any negligence of the North Jersey Home Schoolers Association Inc., its directors, teachers, assistants, volunteers, and chaperones.
I understand and accept in making this release, that I am voluntarily waiving the right to assert a claim, suit, and or demand for any negligently caused personal injury and or property damage against the North Jersey Home Schoolers Association Inc, its directors, teachers, assistants, volunteers, and chaperones, regardless of whether the injury or property damage is said or believed to have been caused in whole or in part by them. I am aware of the risks and hazards involved in the activity.
I understand that this release does not release or waive the right to assert any claims for personal injuries and or property damage against other persons and or entities, as a result of the negligence (in whole or in part) of those other persons or entities, in any combination.
This Release is intended to apply to the participation of the above referenced child in the following event/activity, for which I hereby voluntarily give my consent for them to attend and participate in:
NJHSA Jr/Sr Banquet; & Banquet Afterparty. Please note: the afterparty is not part of NJHSA. The entire event will be held at:
The Grand, 38 Lackawanna Ave, Totowa NJ.
I have read and understand the terms and conditions of this authorization, release and waiver, and represent that I have the authority to execute the same on behalf of the child referred to herein.
